Looking Beyond the Kidneys: An Unexpected Diagnosis
An Ultrasound KUB was performed for a patient presenting with blood in the urine (hematuria).
During the examination, enlarged para-aortic lymph nodes were identified—an unusual finding that raised suspicion for an underlying testicular malignancy. Based on these findings, the patient was counseled about the possibility of a testicular source and was encouraged to undergo a scrotal ultrasound, despite having no specific referral for the scrotum.
The additional evaluation confirmed the suspicion, allowing the diagnosis to be established. The patient was subsequently referred to PGI for definitive treatment. On follow-up after surgery, the patient returned with records confirming the diagnosis.
This case highlights the value of clinical reasoning and a comprehensive imaging approach. Sometimes, recognizing an indirect clue and investigating beyond the initial referral can lead to the timely diagnosis of a serious condition.
