Connecting the Clues: When an Abdominal Ultrasound Reveals the Primary Disease

An Ultrasound Whole Abdomen was performed for a female patient presenting with abdominal bloating and anorexia.

The examination revealed bilateral ovarian masses with ascites, along with multiple liver nodules, raising strong suspicion for metastatic disease. Given the pattern of findings, the patient was questioned about any history of a breast lump. She reported a longstanding breast lump that had never been evaluated.

A focused ultrasound examination of the breast demonstrated an irregular mass. The constellation of imaging findings was highly suggestive of breast malignancy with metastatic involvement of the ovaries (Krukenberg tumor) and liver, prompting urgent referral for further oncological evaluation and tissue diagnosis.

This case highlights how a systematic ultrasound examination, combined with careful clinical history and thoughtful correlation of imaging findings, can uncover the underlying primary malignancy and expedite appropriate treatment.
